It's hard to say conclusively that no part of the ball was inside the box. It's comparable to when you see a corner kick taken with just a slither of the ball hanging over the quadrant. Any part of the ball inside the box should mean it is inside? McHugh's second yellow wasn't the violent close to a straight red that has been suggested, it was a robust challenge but unfortunately that sort of challenge sometimes gets a yellow.
